一、主动、正确告知自己的健康状况，既往史，过敏史和传染病史。

Please avtively offer information concerning  your health status, medical history, allergies and infections history with accuracy.

二、遵守医院工作制度和流程；参与决定治疗方案，共同签署知情同意书。

Please abide by the hospital pollicies and procedures; participate in determining proposed treatment and sign the informed consent forms.

三、配合医务人员为您实施的各项诊疗活动，按医嘱进行治疗、办理监护病房转出、出院或转院手续，珍惜医疗资源，妥善利用、爱护医院各项设施、仪器。

Please follow the protocol concerning the treatment form medical team; discharge or transfer from ICU as advised by the team; treasure and make good use of the medical resources as well as the equipment.

四、遵守医院门禁相关规定、感染控制措施，不得在院内吸烟；不得在医院内大声吵闹，避免影响其他患者治疗和他人权益。

Please abide by pollicies in terms of hospital access control, infection control; please be quiet and no smoking shall be allowed inside the medical building.

五、支付应自行负担的费用；不要求医师提供不实的资料或诊断证明。

Please be responsible for the expense that is supposed to be coverd by you; please do not require the physicians to provide false information of disagnosis certificate.

六、住院和治疗期间请勿向医务人员赠送钱物和宴请。

Please do not donate money or alike to the medical staff during the period of hospitalization and treatment.

七、住院期间，请勿擅自离院或外宿，否则按自动出院处理。

Please do not leave hospital or stay out ocernight without further notifying certain parties during the period of hospitalization; otherwise this act will be regarded as automatic discharge.

八、严禁带重要文件、危险品入院；钱财等贵重物品请自行妥善保管。

Please do not carry important documents or dangerous items with you on your admission to the hospital; keep a close eye on your personal belongings.

九、我院系教学医院，承担教学和科研任务，为提升医学教育水平，培养更多优秀的医务人员，恳请您给予配合。

Please be advised that this a teaching and researching, we appreciate your cooperation in assisting the hospital in raising the level of medical education and training more medical staff.

十、尊重医务人员的劳动和人格尊严。

Please treat medical staff with dignity and respect.
